Making a Claim

To make a claim:

Download and print out the .PDF of the claim form. Download Claim Form Here.
Fill out the top portion (Part 1) of the form. Please make sure you provide all required information.

Bring the claim form to your veterinarian to fill in Part 2 of the form and sign.

Note: Ask your veterinarian if they are using EVE (Online insurance claims filing for veterinary hospitals)

Return your claim form, detailed paid invoice and complete medical history
via fax or mail to:
